Transition to Work assists young people to find the best mix of activities and services to help them achieve their goals.

Moving can be difficult. For Jacob, it meant that he had to resign from his job in order to stay with his family. Once settled in his new home Jacob struggled to find employment and needed some help. Through the Transition to Work (TtW) program, Jacob was able to build the skills and confidence he needed to find a new job and continue his education.

TtW helps 15–24-year-olds into work (including apprenticeships and training) or education. Participants receive intensive, pre-employment support to develop practical skills, connect with education or training, find local job opportunities, and connect with relevant local community services.

Jacob’s TtW Provider identified that Jacob would be an asset in the care and support sector due to his qualities and personality traits. Jacob wanted to make a difference in people’s lives and this opportunity was a perfect fit. Through TtW, Jacob was able to obtain a Police Check and a Working with Childrens Check to enable him to gain employment as an individual support worker.

Jacob’s employment has been secure since his commencement and with the support of TtW, he also enrolled in a Certificate IV in Disability. This Certificate will help him to gain the knowledge and experience he will need to further progress his career. While studying he will continue to make a real impact in the community by providing support to those who live with disability.

Through TtW Jacob has found a career that truly makes him happy, and one he sees a bright future in. He is a hard-worker and is recognised for the positive contribution he makes to the lives of the people he cares for.
